
Pro Fit Lifestyle is committed to highest quality patient care. Our accredited Exercise Physiologists are trained to develop customised exercise therapy programs to suit each patient’s exact needs, helping them with the long-term management of their condition.
Importantly, our programs complement and enhance the patient’s overall treatment plan, enabling them to reach their lifestyle goals with safety, support and confidence.
As the referring GP or Specialist, Pro Fit Lifestyle advises you on the activities prescribed to your patient, and provides you with regular updates on their progress throughout the program.
Referrals
Pro Fit Lifestyle accepts the following types of referrals:
1. Enhanced Primary Care (EPC) Program
The EPC program is for patients with chronic conditions and/or complex medical needs.
To be eligible for the program, patients must have been billed either a GP Management Plan (item 721 or review item 725) or a Team Care Arrangement (item 723 or review item 727) in the previous 2 years.

2. Type 2 Diabetes Group Services Programs
The Type 2 Diabetes programs are for patients who have been diagnosed with Type 2 Diabetes (items 81100 – 81125). To be eligible for a program, patients must have been billed a GP Management Plan (item 721 or review item 725) in the previous 2 years.

3. Department of Veterans Affairs (DVA) Program
The DVA program is for patients who hold either a Gold or White Repatriation Health Card.
4. Private Health Insurance
Many private health insurers allow patients to claim a certain amount each year for Exercise Physiology services. Please see the Private Referral Form for further details.
5. Private patients
Pro Fit Lifestyle also offers Exercise Physiology services for private patients. Please see the Private Referral Form for further details.
